What we do


Our Planned Care (District Nurses) at The Royal Wolverhampton NHS Trust are a vital part of your local NHS team, dedicated to providing high-quality care right in your own home or residential care facility here in Wolverhampton. We understand that being able to receive care where you’re most comfortable can make a huge difference to your well-being.  Our teams work across four areas of Wolverhampton, bringing essential care directly to people who are unable to leave their homes due to illness or disability. This means you get the support you need, helping you to live as full and independent a life as possible.

What Does Our Planned Care Team Do?

Our Planned Care team is made up of skilled registered nurses, nursing associates, and healthcare assistants. They have a wide range of expertise to deliver planned treatments and support within the community.

They’re here to help you manage long-term conditions, providing the clinical skills and knowledge you need to maintain your independence. By supporting you at home, we aim to prevent unnecessary hospital stays, allowing you to stay in familiar surroundings and continue with your daily life as much as possible.

Where we are based?


District Nurses are based at the University of Wolverhampton Science Park, Technology Centre Building, WV10 9RU.
Operating hours are: 8.00am -8.00pm, 7days a week, including Bank Holidays.

How to access the service


Our Planned Care service is designed to support adults in Wolverhampton who need nursing care but are unable to leave their homes. To access our service, you must be over 18 years old, live in Wolverhampton, and be registered with a GP in Wolverhampton. We understand that getting to appointments can be difficult when you’re unwell, so our team comes to you.

We provide a range of nursing interventions in your home. This includes help with administering medications such as those given via PEG (a feeding tube), intramuscular (IM) injections, or subcutaneous (SC) injections. If you are a diabetic patient on insulin and are unable to administer it yourself, our nurses can help with this too. We also offer care for various types of wounds, including chronic wounds, pressure ulcers, and simple wounds, as well as providing compressive therapy and ankle-brachial pressure index (ABPI) assessments.

Our team can also assist with urinary catheter care and maintenance, and provide support with bowel care. For those nearing the end of their life or with unstable palliative care needs, including the management of syringe drivers, our nurses offer compassionate and skilled support. Please note that for stable palliative patients, support is provided via the Virtual Ward service, and intravenous (IV) medication administration is managed by our Hospital at Home team. We also manage various types of drains.
