Children's Services (0-19)

The 0-19 Healthy Child Programme (HCP) is a universal service available to all children from birth until the age of 19 in Wolverhampton. It consists of the following four key services:

  • Health Visiting
  • School Nursing
  • Partnering Families Team (PFT)
  • Health Inclusion Team (HIT)

The early years of childhood, through to adolescence, are vitally important to growth, learning, overall development, and mental health.

We aim to ensure that every child and family gets the best possible start they need, to lay the foundations of a healthy life.

The service is based at:

Phoenix Health Centre
Parkfield Road
Wolverhampton
WV4 6ED

Services are also delivered from:

  • Community Health Centres, including: the Gem Centre, Bilston Health Centre, Pendeford Health Centre, Primrose Health Centre, Warstones Health Centre and Whitmore Reans Health Centre
  • Wolverhampton schools, including: primary, secondary, special schools and pupil referral units
  • Youth Justice Service
  • Wolverhampton Family Hubs
  • Patients’ homes

Who is this service for?

Pregnant women, children and young people from the age of 0 to 19, and their families.

How to access these services

We accept referrals from anyone, including professionals, children, young people and family members.

Confidentiality

Unless you give permission, the 0-19 team will not share anything you say with anyone else, such as parents, teachers, or similar professionals – except in extreme circumstances. For example, if the health, safety or welfare of you/your young person or someone else is at significant risk.

If we did need to tell anyone about something you’ve told us in confidence, we would always try and speak with you first. For our safety and yours, we hold any private information you share with us in electronic records, which can also be seen by other healthcare professionals. These professionals, which can include your doctor, all follow the same confidentiality. Records are kept for future use.
