The Health Visiting Service (Children)

What we do

The Health Visiting Service is a universal service which supports local families and their baby/child from 0-5 years. Service delivery is underpinned by evidence-based practice and sets out to enhance health and reduce health inequalities. Working closely with parents, we support and provide informed advice from the antenatal period through until your child starts school.

We are made up of Health Visitors, Public Health Associates, data and admin staff.  We offer support through a combination of home visits, group work and clinics.

Where we are based

The service is based at:

Phoenix Health Centre
Parkfield Road
Wolverhampton
WV4 6ED

Our services are also delivered from:

  • Community Health Centres including the Gem Centre, Bilston Health Centre, Pendeford Health Centre, Primrose Health Centre, Warstones Health Centre and Whitmore Reans Health Centre
  • Wolverhampton Family Hubs
  • Patients’ own home

How to access the service

You can receive direct advice from our Health Visitors and Public Health Associates through our Single Point of Access, open Monday-Friday, 9:00am to 5:00pm.

We work closely with the multi-disciplinary team, including health professionals, nurseries, social care and voluntary organisations.

Referrals are accepted from anyone, including professionals, children, young people and family members.

The targeted element of the service includes additional support for vulnerable populations, Special Educational Needs and Disabilities (SEND), developmental delay, safeguarding and parental mental health. The targeted offer will vary from family to family depending on their individual needs.
