We’re dedicated to improving patient flow by allowing you to avoid hospital admissions or return home sooner to recover, with our services available to those registered with a GP in Wolverhampton or South Staffordshire.

Our dedicated team of nurses, Advanced Nurse Practitioners (ANPs), prescribing pharmacists, and speciality consultants monitor your condition daily, often providing technology like remote blood pressure monitors for observations.

We accept referrals 24/7, admitting adult patients seven days a week until 10 pm, and paediatric patients until 7 pm. All referrals are triaged quickly, ensuring a care plan is developed during a home visit within 24 hours.

If you meet the basic referral criteria of being medically stable but requiring ongoing acute care that would otherwise need a hospital bed, your GP or healthcare professional can refer you to one of our many pathways, including Acute Medicine, Diabetes, Frailty, Heart Failure, and Paediatrics.
