What we do


Our Rapid Access to Social Care (RASC) Team is here to help you get home from the hospital sooner, or even avoid a hospital stay altogether during a crisis. We do this by providing a short-term, temporary package of social care support. This support continues until social services can arrange a long-term care plan that’s just right for your needs. Our main goal is to help you stay safe and comfortable in your own home.

The Rapid Access to Social Care Team (RASC) supports you to be discharged sooner from hospital or avoid hospital admission in a crisis situation by providing a short-term, temporary package of social care. This support continues until social services can arrange a long-term care plan tailored to the patient’s needs.

How We Can Support You

This team also aligns with Planned Care to collaboratively support with the personal care needs of patients at the end of their life. The support they offer ranges from:

  • Personal care – including mouth care and skin care.
  • aSSKINg prevention.
  • Meal preparation.
  • Medication administration (under certain criteria).
  • Health promotion.
  • Mobility support.
  • Accessing equipment.
  • Access further services within the MDT.

How to access the service


Referrals are made via Care Coordination on 01902443322.

We accept direct referrals from patients and carers.

Service operates: 8.00am – 10.00pm, 7 days a week.
