What we do
Our Rapid Access to Social Care (RASC) Team is here to help you get home from the hospital sooner, or even avoid a hospital stay altogether during a crisis. We do this by providing a short-term, temporary package of social care support. This support continues until social services can arrange a long-term care plan that’s just right for your needs. Our main goal is to help you stay safe and comfortable in your own home.
How We Can Support You
The RASC team also works closely with our Planned Care service to provide essential personal care for patients at the end of their life, ensuring you receive dignified and compassionate support. The help we offer covers a range of your daily needs, including:
- Personal care, such as mouth care and looking after your skin.
- Support with pressure ulcer prevention (aSSSKINg prevention).
- Preparing meals for you.
- Help with medication administration (under certain conditions).
- Health promotion advice to help you stay well.
- Mobility support to help you move around safely.
- Helping you to access necessary equipment.
- Connecting you with further services within our wider healthcare team.
