Planned Care (District Nurses) are an integral part of the NHS Workforce who make a meaningful impact each day to the lives of people they visit at their own homes and in residential care facilities across Wolverhampton. Across four geographical localities, they deliver essential care to housebound patients, ensuring both quality care provision and an enhanced quality of life.
What Does Our Planned Care Team Do?
Our Planned Care service is designed to support adults in Wolverhampton who need nursing care but are unable to leave their homes. To access our service, you must be over 18 years old, live in Wolverhampton, and be registered with a GP in Wolverhampton.
Planned Care is a dedicated team which consists of registered nurses, nursing associates and healthcare assistants all equipped with a diverse set of skills to provide planned treatments in the community. Their clinical skills and knowledge enables them to support people to manage their long-term conditions and maximise their independence, this in turn preventing unnecessary admissions to hospital.
We provide a range of nursing interventions in your home. This includes help with administering medications such as those given via PEG (a feeding tube), intramuscular (IM) injections, or subcutaneous (SC) injections. If you are a diabetic patient on insulin and are unable to administer it yourself, our nurses can help with this too. We also offer care for various types of wounds, including chronic wounds, pressure ulcers, and simple wounds, as well as providing compressive therapy and ankle-brachial pressure index (ABPI) assessments. Our team can also assist with urinary catheter care and maintenance and provide support with bowel care. For those nearing the end of their life or with unstable palliative care needs, including the management of syringe drivers, our nurses offer compassionate and skilled support. We also manage various types of drains.
