What we do

Our dedicated team is here to support you in a number of ways. We’re a multidisciplinary team, which means we have different healthcare professionals all working together with one main goal: to help you stay at home and live as independently as possible. Whether you’re just leaving the hospital and need some extra support to get back on your feet, or you need help to avoid a hospital stay or needing social care, we’re here for you.

Our main focus is to help you regain your independence, provide you with the rehabilitation you need, and promote your recovery in the comfort of your own home. We want to help you get back to doing the things you love as quickly and safely as possible.

Our team includes experienced senior nurses, staff nurses, physiotherapists, occupational therapists, senior support workers, and rehabilitation assistants. Each member brings their own unique skills to support your journey to recovery and independence.
