One Health and Care

In the past, patients have been asked the same questions about themselves when visiting different health and care providers – such as a GP or a hospital.

This is because different providers used different systems to record your details. These systems weren’t linked, so the organisation looking after you couldn’t see information held by others.

The introduction of One Health and Care – a confidential NHS shared care record for the Black Country – allows professionals to view information that is held by other parts of the health and social care system.

It sits on top of existing digital records systems, which means it is only visible to those who are directly involved in your care, and professionals only see information that is relevant to their role.

Learn more about One Health and Care, how it could benefit you, and how partners are keeping your information safe, by visiting the Black Country Integrated Care System (ICS) website.
