What we do
Our Community Anticoagulation Care Team (ACT) is a dedicated team of nurses here to help manage your blood thinning medication. We understand how important it is to keep your treatment on track, and we’re here to make it as easy as possible for you.
You can visit our team at Pendeford Health Centre, and we also operate from other convenient locations across Wolverhampton. We’re available Monday to Sunday, from 8:30 AM to 4:30 PM. If you’re unable to leave your home due to illness or mobility issues, we can even provide a home visit service to ensure you still receive the care you need.
How we can support you
Our nurse-led service offers a comprehensive range of support to ensure your anticoagulation therapy is managed safely and effectively. We provide:
Nurse-led clinics for new patients to get you started on your treatment.
Nurse-led dosing and review of your anticoagulation therapy to ensure it’s always right for you.
Help with switching to an alternative anticoagulation therapy if needed.
Home visits for housebound patients, for both starting and monitoring your anticoagulation therapy.
A self-testing programme, giving you more control over your treatment.
Expert support from a Consultant Haematologist for more complex cases.
Complex caseload coordination and management by our Senior Sister, ensuring seamless care.
Nurse prescribing for your convenience.
A comprehensive education package for all new patients or those switching therapies, so you fully understand your treatment.
A dedicated nurse dosing line to provide dosing support when you call in with your INR results.
A telephone dosing line specifically for new Cannock patients until your treatment is stable, accessible via the Cannock Single Point of Access.
Support to avoid hospital admissions by administering Vitamin K or Low Molecular Weight Heparin (LMWH) when necessary.
Management of your anticoagulation therapy before elective invasive procedures.
Initiation of Warfarin or DOAC treatment to begin your therapy.
Our team also works closely with other services within The Royal Wolverhampton NHS Trust (RWT) to ensure that if you’re an inpatient, you receive expert advice and care regarding anticoagulation and VTE (venous thromboembolism, or blood clot) prevention.

Getting referred to our Community Anticoagulation Care Team is straightforward. Your healthcare professional can make a referral for you by contacting Care Coordination on 01902 443322. We also accept internal referrals from other NHS services.
Who Can Our Service Help?
Our service is primarily for:
Adults aged 18 and over who are registered with a Wolverhampton GP and have a Wolverhampton postcode. This includes patients who have just started a new anticoagulation treatment or have switched to a different one.
We can also accept referrals for young people from 16 years of age if they are referred by the Wolverhampton paediatric team.
If you have a Cannock GP and a more complex medical situation, we can offer a limited service. Referrals for these cases come through Care Coordination and the Cannock Single Point of Access (SPA).
Who Our Service Cannot See
Unfortunately, there are a few situations where our service might not be the right fit:
Patients under 18 years old, unless they are 16 or 17 and referred by the Wolverhampton paediatric team.
Patients who are not registered with a Wolverhampton or Cannock GP.
If your GP practice already runs its own enhanced service scheme for managing anticoagulation, they will continue to look after your care. Care Coordination can advise you if this applies to your GP.
