What we do
As an Urgent Community Response Service (UCR), we are focused on improving the life chances and health outcomes of the diverse population of the communities we serve. We are passionate about supporting our patients and endeavour to enable them to remain at home when they are having a crisis, both of a clinical or social nature.
The Urgent Community Response Service (UCR) is a nurse led team of highly skilled nurses and Advanced Clinical Practitioners (ACP’s) with medical Consultant cover five days a week. This gives us access to a wide range of interventions and treatments such as requesting blood tests, x-rays and ultrasounds, and we have nurse prescribers available 24 hours a day, 7 days a week.
By understanding the needs of our communities, we can use our clinical expertise to significantly contribute to the development of pathways that continually improve the quality of care.
Urgent 2 Hour Response:
A two-hour response is to be delivered to a person at risk of admission (or re-admission) to hospital due to a crisis and there is a likelihood that they will attend hospital within the next two to 24 hours without intervention to prevent further deterioration and to keep them safe at home.
The service provides care to people in the place they call home which helps to avoid hospital admissions and enables people to live independently for longer. Older people and adults with complex health needs who urgently need care gain fast access to a range of health and social care professionals within two hours. This includes access to physiotherapy and occupational therapy, medication prescribing and reviews, and help with staying well-fed and hydrated.
Where a person does not require a response within two hours, they should receive care within the timescales they require and be supported to remain safely at home with appropriate services.
